The equation of a straight line in slope and y-intercept form is:
$y=mx+c$ where $m$ is the slope of the line and $c$ is the y-intercept.

In this example we have $y = (1/2)x + 3$

Note that this equation is already in slope and y-intercept form with $m=1/2 and c=3$
